What is a DAP note?
A DAP note is a three-part progress note used across behavioral health. It records one session in a fixed order, so anyone reading the chart later can find what happened, what it meant and what comes next.
- Data is the factual record of the session: what the client reported, what you observed (mood, affect, engagement, risk statements) and the interventions you used.
- Assessment is your clinical reading of that data: the client's progress, changes in symptoms, how they responded to the work and any impressions you formed.
- Plan is what happens next: homework, goals you set, referrals and when you will meet again.
DAP is shorter than a SOAP note because it folds the subjective and objective parts of the session into one Data section. Many counselors find that matches how a talk-therapy session actually unfolds. Our DAP notes guide covers the format in more depth.
How to write a good DAP note
- Write it soon after the session. Details fade quickly. A few lines of shorthand straight after the client leaves are enough for the generator to work with.
- Keep Data observable. Use the client's own words where they matter, and describe what you saw rather than labels: “tearful when discussing her father” says more than “sad”.
- Make Assessment yours. State progress in terms you can measure: symptom frequency, scale scores, sleep, attendance. Only link progress to a treatment goal when you made that link.
- Make Plan concrete. Name the homework, any referral, and the date or interval of the next session.
- Always document risk. If you asked about suicidal or homicidal ideation, record the answer, including negatives such as “denies SI”.
How this DAP note generator works
Paste your notes or press Speak instead, then generate. The generator only uses what you wrote: it will not invent symptoms, scores, diagnoses or homework, and any section your notes do not cover reads “Not documented” so you can see what is missing. It leaves out names and other identifiers even if you include them, keeps negations like “no SI” intact, and never connects a finding to a goal unless you did. Read the draft, fix anything that is off, and copy it into your record.
Prefer a different structure? The same generator writes BIRP notes and SOAP notes. For a wider look at documentation formats, see our progress notes guide.
From session notes to no notes
This page saves you the formatting. PatientNotes saves the typing too: it records the session, with consent, and writes the DAP note in your own template while you stay with your client. See how it compares with other therapy notes apps.
