Is this SOAP note generator really free?

Yes. You can generate notes without paying or signing up. Each visitor gets a small number of free notes per hour and per day so the tool stays fast for everyone.

Do I need an account?

No. Open the page, type or speak your notes and generate. An account is only needed if you want PatientNotes to record visits and write notes for you.

Is it HIPAA compliant? Can I enter patient information?

Don't enter names, dates of birth, record numbers or other identifiers. Your text and audio are sent to our AI providers to write the note, and PatientNotes doesn't store what you enter, the recording or the result. Treat this page as a drafting aid for de-identified shorthand. For real patient records, use PatientNotes itself, which is built for patient data and signs a Business Associate Agreement (BAA) with paid customers.

What's the difference between a SOAP note and a DAP note?

A SOAP note has four parts: Subjective (what the patient reports), Objective (what you observed or measured), Assessment and Plan. A DAP note folds the first two into Data, then Assessment and Plan. DAP is common in therapy and counselling; SOAP is the default almost everywhere else.

Can I dictate instead of typing?

Yes. Press "Speak instead (60 sec)", allow the microphone, and talk for up to a minute. The transcript fills the text box so you can correct it before you generate. If your browser blocks the microphone, you can still type.

Which specialties does it support?

General medicine and primary care, dental, therapy and counselling, chiropractic, physical therapy, nursing and veterinary. Each one uses the wording and structure clinicians in that field expect.

How accurate is the AI note?

It is a draft. The generator only uses what you wrote and is told not to add findings, vitals, doses or diagnoses you did not mention. Any section your notes do not cover reads "Not documented". Always review the note before it goes into a record; you remain responsible for its content.

How is this different from PatientNotes?

This page turns notes you have already typed into a SOAP note. PatientNotes records the whole visit on your phone or in the browser, writes the note in your own template, suggests ICD-10 and CPT codes, and keeps your notes in one secure place.
